In order to obtain the hospital's consent to the study, the researcher shall submit to the Scientific Institute (Pilsoņu Street 13, Building 21) in computer or clearly legible handwriting completed in block letters:

• Application for a hospital permit
• Confidentiality statement
• Study description, summary
• Other documents required for the study, such as a statement of the medical equipment to be used in the study
• Written permission of the University or other competent ethics committee (permission of the RSU Ethics Commitee)
